WesleyLife Hospice honored for military veterans affiliation accent

July 15, 2024 | By
The National Hospice and Palliative Care Organization and the advocacy organization We Honor Veterans have recognized WesleyLife Hospice as the nation's first hospice partner to complete the Community Nursing Home Initiative pilot program.

Developed with the U.S. Department of Veterans Affairs, the program works to improve hospice care for terminally ill veterans in Veterans Administration-contracted communities for older adults, engaging the communities to provide specialized education to help team members care for veterans at the end of life.

As a Level 4 partner of We Honor Veterans — expected to reach the top tier, Level 5, next year — WesleyLife Hospice has fulfilled program requirements such as the implementation of veteran-specific intake and screening procedures, veteran hospice volunteer recruitment, and community partner engagement. Since its affiliation with the program in 2014, WesleyLife Hospice has impacted the lives of countless veterans and their families.

Congratulations to the WesleyLife Hospice team — part of the WesleyLife at Home team, pictured here — on this national accolade!
